 New Patient-Centered Medical Home Benchmarks

The patient-centered medical home (PCMH) model of care — an approach embraced by thousands of healthcare organizations — focuses on improving care quality and reducing costs for the chronically ill. According to a 2004 report in the Annals of Family Medicine, if every American had a medical home, healthcare costs would decrease by 5.6 percent, resulting in national savings of $67 billion per year and improved care.

Medical Home Awareness
Over 90 percent of organizations surveyed in a recent Healthcare Intelligence Network analysis (221 respondents) are familiar with the term “medical home” yet only 37.8 percent of those familiar with the term had established PCMH programs for their members and/or patients.

However, illustrating the growth in this approach to healthcare, nearly half — 44.8 percent — of respondents without a PCMH approach do plan to establish medical homes for their members and/or patients within the next 12 months, according to the new HIN research report, Model Medical Homes: Benchmarks and Case Studies in Patient-Centered Care. Model Medical Homes: Benchmarks and Case Studies in Patient-Centered Care is a landmark publication that documents the healthcare industry's adoption of the patient-centered medical home model of care. This exclusive 65-page report analyzes the responses of more than 220 healthcare organizations to HIN's 2009 Industry Survey on the Patient-Centered Medical Home Model.

On average, respondents note that it took 12 to 18 months to convert practices into PCMHs (52.7 percent). Another 21.8 percent said it took less than a year to transition to PCMH, while 14.5 percent say it took them more than two years, found the study.
